New patient appointments
 
 

As a New Patient, please register via this website at the drop-down menu listing for New Patients under Patient Services. This gives us some basic information about you as well as a brief medical history. It is all on a SECURE site with encryption when sent to our office, and only viewed by Dr. Pomeroy or his staff.

* If you are a parent registering for a child, please register as if you were the child (i.e. use their name, date of birth, etc) in the registration sections.  Otherwise we think the appointment is for you. 

 *With any line in the registration process, if it is *Required, and it does not apply to you, put something in there. "n/a" or "none" or "asdflkjasdf" will work. If it is left blank, you will be asked again to complete the page. IMPORTANT - If you have no health insurance, enter "none" in the first box, and put some numbers or letters into the other boxes so that the program allows you to complete registration.

If your concern is about AD/HD issues for yourself or your child- diagnosis, medications, other treatments 

  • Please make a One Hour appt, and expect to make a follow-up within 6 weeks. Our time at your appointment can best utilized if you complete some additional history forms and rating scales. These can be done online from the Symptom Assessment section of the Patient Services dropdown list at the Home Page, or completed at our office using the computer kiosk in the waiting area.  History in this format is entered directly into your chart, and saves me time sorting through various reports and records.
  • BRING PAST RECORDS of any "educational testing" done at school for IEP placement, any AD/HD rating scales, etc from another doctor's office, or with a private psychologist or at school; these records are VERY important. Please sign a Release or Exchange of Information for those to be sent here, preferably to arrive prior to your appointment. You bring them to the first appointment, but I will need time between the first and second appts to review the material in depth.

If you wish to make an appointment for evaluation of a mood problem like anxiety or depression, please make a One Hour NewPatient appointment via our website using the link to Appointment Quest (automatic by clicking on Appointments under Patient Services); please register first so we have a way to contact you and have some basic medical history. In addition, completing the Depression/Anxiety intake section from the Symptom Assessment dropdown will also help. My staff will call you to prompt completion of this information if necessary, prior to confirming your appointment time. 

PLEASE ARRIVE AT LEAST 15 MINUTES PRIOR TO YOUR APPOINTMENT TIME so we can verify some information, have you read and sign  the basic consent forms and HIPAA forms, and attend to insurance or payment details.
