Rating scales of behaviors, or traits, or thought processes are among the tools doctors and psychologists use to get a picture of how someone's mind works. These are very helpful in sorting through the possible explanations for a group of traits or ways of behaving; in medical terms helping to "arrive at a diagnosis".  The surveys do not provide the answer, but they can show the likelihood of AD/HD being present, or not likely to be present. The same for issues which may complicate the picture such as depression, anxiety, bipolar depression. 

Now it will be much easier to complete these scales for Initial Visits and also for periodic monitoring of the effectiveness of treatments and overall approach to AD/HD.  They can be accessed in two ways, for different groups of people.

Adults and Teens access these via the SYMPTOM ASSESSMENT line of the PATIENT SERVICES dropdown section.  Use the password "imh" to get going.

  • After completing the registration information with just your name and birthdate, you are taken into the survey.
  • Questions appear one at a time on the screen. You may recognize some from other surveys done here or other places.
  • Answer the questions based on your experience - over the past 6 months for Iniital intake;  over 2-3 weeks for status checks. ANSWER ALL QUESTIONS BASED ON YOUR ACTUAL MEDICATION USE AT THE TIME, NOT HOW YOU FELT BEFORE TAKING MEDS !!
  • When all the questions are done, you click "submit" to send the encrypted data over the Internet to our servers at BHNW, where it is unscrambled and placed in your file, ready for your next visit.
  • THIS IS A SECURE SYSTEM, ONLY STAFF HERE AT ADD Center of Bellevue WILL BE ABLE TO SEE THE ANSWERS AND SCALE RESULTS.

Children with possible AD/HD have a different set of questions for parents and teachers to answer. These are found on the "Links" section of our drop-down list from Patient Services. Not yet linked electronically to us, so print out a copy of the NICHQ Parent survey, and at least one Teacher survey. If your child has 2 or 3 teachers, ask all to complete it.  For kids in grades with many teachers, one from a morning class and one from an afternoon class is best. Any Special Ed teacher is also great to have input from. Print out the surveys, complete and either mail them in, FAX them here if you wish (fair warning - this may not be a secure communication according to HIPAA regulations), or bring them with you to your visit.

I am excited to offer this convenience to you, and also to make use of it for ongoing tracking of your progress even between visits. I think it can be a valuable tool for me to best help you keep on track and stay in balance as you make progress along your path of adventures in living well with AD/HD.
