ADD Center of Bellevue does not have contracts with any health insurers other than Premera and Regence. That contract specifies that we must bill Premera or Regence first, and accept their determination of how much gets paid and by whom, for their clients.

If you have a different health insurance, you can staple the billing form we will give you at the end of your visit to the Claim Form from your insurer, and submit it to them per their instructions. They will review the claim, and make payment to you according to the rules of your health plan.

Our billing form contains all the items insurers require, including dates of service, diagnosis codes (ICD-9 system) and charge codes ("cpt coding"), and your insurance information.
